Welcome to Miyasaki Dental. Completing your new patient forms before your first appointment gives us time to review your dental and medical history, medications, and any concerns you would like to discuss. It also means less paperwork when you arrive and more time for your visit.
Getting Started
Download and complete the new patient packet before your appointment. Please fill out the forms as thoroughly as possible so we have the information needed to understand your health history and provide appropriate care.
If you have trouble accessing the forms or are unsure how to answer a question, call our front desk. Our team can help you with the paperwork before your visit.
Your Privacy Matters
The personal and health information you provide is handled in accordance with HIPAA privacy requirements. We take appropriate steps to protect your information and do not disclose it without your authorization except when permitted or required by law.
What to Bring to Your First Visit
Along with your completed new patient forms, please bring:
A valid photo ID
Your dental insurance information, if applicable
A current list of medications and supplements you take
Information about relevant medical conditions or allergies
Details about previous dental treatment that may be helpful
Any questions or concerns you would like to discuss with us
If you have recent dental records or X-rays from another office, let our team know so we can determine whether they would be helpful for your care.
What to Expect at Your First Visit
Your first appointment gives us an opportunity to understand your oral health, concerns, and treatment goals. Depending on the reason for your visit, we may:
Review your dental and medical history
Discuss current symptoms or concerns
Examine your teeth, gums, bite, TMJ and screen for oral cancer.
Take diagnostic images when clinically appropriate
Review our findings with you
Discuss preventive care or treatment options if needed
We won’t recommend the same care for everyone. Your exam findings, health history, symptoms, and priorities help guide the next steps.
A Multigenerational Sacramento Dental Practice
Miyasaki Dental has served the Sacramento community for more than 60 years. Dr. Michael Miyasaki now practices alongside his daughter, continuing the practice’s multigenerational approach to dental care.
Our “Dentistry 2.0” philosophy emphasizes prevention, patient education, and appropriate use of dental technology rather than waiting for problems to become more advanced. Insurance, Patient Plans, and Payment Options
We accept PPO dental insurance and can help you understand the benefits available under your plan. Miyasaki Dentalalso offers separate in-house Patient Plans for patients who prefer an alternative way to budget for preventive care.
For larger treatment costs, third-party financing is available through Cherry and CareCredit®. Our front office team can explain the available payment options and estimated costs before treatment.
